Since 2016, the Pharmacy Integration Fund has funded and tested several operational service models throughout England that can be used to enhance community pharmacy’s role in urgent care provision.

Using the evidence from these pilots, the Community Pharmacist Consultation Service
(CPCS) is being commissioned as an Advanced Service from 29 October 2019, with the
expectation that additional strands to the service will continue to be tested and developed in the future. At the time of service commencement, the referrals to community pharmacies will be made by NHS 111 or Integrated Urgent Care Clinical Assessment Services (IUC CAS).

CPCS replaces the NHS Urgent Medicines Supply Advanced Service (NUMSAS) and the Digital Minor Illness Referral Service pilots; these services will continue to be commissioned until 28th October 2019.

How can I register for the CPCS service?

If you want to provide the service, you need to register to use the Manage Your Service (MYS) portal on the NHSBSA website.

You can find further MYS guidance on the LPC’s webpage.

If you want to register multiple pharmacies at the same time, please email  nhsbsa.mys@nhs.net.

Registrations are open from 1 September 2019. The service will be live from 29 October 2019. Any claims made for 29 to 31 October 2019 will be processed in November.

How much transition payment can I get?

If you sign up to CPCS between 1 September 2019 and 11:59pm on 1 December 2019, you’ll receive a £900 transition payment. If you sign up between 2 December 2019 and 11:59pm on 15 January 2020, you’ll receive £600.

Any registrations after 16 January 2020 will not receive a transition payment.

How much does the pharmacy get paid to provide the CPCS service?

When a pharmacy provides the service, they can claim a £14 fee per completed consultation.

How do I submit a claim for payment?

You will need to use either PharmaOutcomes or Sonar Health to receive referrals and process claims. The MYS portal will be automatically prepopulated with the claim information. You will then be able to verify and submit claims each month.

EPS tokens for exempt patients must still be sent to NHSBSA to be scanned and kept for Post Payment Verification (PPV) purposes.

Which medicines can I supply under the urgent supply strand of the CPCS?

Technically, any medicine or appliance that can be supplied on an NHS prescription – in line with the provisions of the Human Medicines Regulations – and has previously been prescribed on an NHS prescription can be supplied as part of the service, where the pharmacist determines that it is appropriate to do so.

Claims to the NHSBSA for reimbursement for the cost of urgent medicines or appliances supplied will be made via the CPCS IT system. The NHSBSA can only process these electronic claims; there is no paper claims process available for the service and electronic claims must use dm+d codes. It is possible that in some circumstances, a medicine or appliance may be requested that is not listed in dm+d, e.g. specials. Where it is deemed appropriate to make an emergency supply, but the item is not listed in dm+d, it will be necessary for the patient to be referred to another service which can issue an urgent prescription, such as the GP OOH service.

I have received an urgent medicine supply referral for a patient, however their GP practice is still open. What is the best way to proceed?

Patients should be advised to contact the GP practice if this is practically the most appropriate option to obtain their medicine or appliance. This may not be possible if, for example, the patient is away from home and unable to access their GP practice or where the GP practice will not be able to issue a prescription in a timely manner, such that the patient can get that prescription dispensed in time for their next required dose of the medicine. The pharmacist will need to assess each individual request and the circumstances of the patient to determine whether it is appropriate to make a supply via the CPCS.

Can I refer patients to NHS 111 to access the minor illness strand of the CPCS?

No. Patients presenting in the pharmacy seeking self-care advice should be dealt with using the normal pharmacy procedures, in line with the requirements set out in the Support for self-care service.

Can I refer patients to NHS 111 to access the urgent supply strand of the CPCS?

Yes, in certain circumstances. If a patient requests an emergency supply from a pharmacy and the request meets the requirements of the Human Medicines Regulations and the pharmacist believes it is appropriate to make a supply, but the patient cannot pay for the emergency supply as a private service and it is also not practicable for them to obtain a prescription from their general practice before they next need a dose of the medicine, it may in those circumstances be appropriate to refer the patient to NHS 111. The patient should be advised to call NHS 111 so the call adviser can determine the most appropriate course of action for the patient, which may include offering them a referral to the CPCS or potentially an appointment with the GP OOH service.

Standard Operating Procedure (SOP)

Pharmacy contractors providing the CPCS must have a Standard Operating Procedure (SOP) in place for the service. A template SOP has been written by members of the Urgent Care Delivery and Implementation Group. Contractors can amend and adopt this SOP for use in their pharmacy if they wish.
